Illinois law requires the following of our incoming students:
All 9th grade students and students new to Illinois schools must provide a current physical and immunization record.
All 9th grade students also must have proof of a dental examination.
Students entering from out of state must provide a current physical and immunization record.
All 12th grade students will need to provide proof of having had the meningococcal conjugate vaccine on or after their 16th birthday.
Please make an appointment with your doctor or with the Department of Health today! We do not want your student to miss out on any learning this school year!
